Google Apps Script Dev: Build a Custom Branded Survey That Feeds Into Google Sheets

I’m looking for someone to build a custom online survey for me that looks and behaves like this existing survey:

I do NOT need the underlying software recreated.

I simply need a new survey built using the same or a very similar setup, with a clean branded interface, one-question-at-a-time flow, smooth navigation, and all responses automatically collected in a Google Sheet.

What I need:

• Build the survey using Google Sites, Google Apps Script, or whatever appropriate Google-based setup creates the same experience

• Match the general look, feel, and behavior of the example survey

• Make it mobile-friendly

• Add all survey questions and answer options I provide

• Include written response fields where needed

• Include multiple choice, single-select, scales, and other question types as needed

• Allow users to move forward and backward through the survey

• Include a progress indicator if appropriate

• Create a clean completion/thank-you screen

• Send every response into a Google Spreadsheet

• Organize the spreadsheet so I can easily review each person’s complete set of answers

• Build everything inside or transfer everything into my Google account so I own and control it

The main goal is simple:

I want a polished survey that feels much better than a standard Google Form, while still giving me all the results in Google Sheets.
